1. Print handpiece repair form. Then fill out the form as completely as you
can and save one copy for your records.
2. PROTECT instruments by cushioning them before
shipping.
3. Include a copy of the repair order form with any special
instructions.
4. Print the prepaid shipping label, attach the label to your
own secure packaging. Securely seal package and mail.
